LONG-TERM CARE COMMUNITIES: ST. VINCENT'S PLACE, RAPHAEL WING, ST. ANTHONY'S WING & EDGAR HALL
Access to the Long Term Care (LTC) at Providence Place including Guardian Grove, is through the Five Hills Health Region Access Centre at (306) 692-6596, or toll free 1-866-211-5696. A community coordinator will be assigned to you. Once it has been determined that you may be a LTC candidate, the coordinator will make a presentation on your behalf to the Regional Continuing Care Access Committee (single point of entry) for a determination of your level of care. If your care needs give you a designation of Level 3 or 4, arrangements will be made for admission to the LTC facility of your choice.
Each of the four long-term care communities located on the second floor of Providence Place, were specially designed and constructed to meet the physical, social and privacy needs of persons requiring level 3 and 4 care. Private rooms, each with it's own window and ensuite washroom, may be decorated with the resident's own photographs, pictures and small furnishings.
Determined to add life to years, not just years to life, Providence Place subscribes to a social model of care. We take care and pride in ensuring life within our facility is as homelike and normalized as possible. Residents, unless they are ill, get out of bed and dress in their own clothing each day. They eat in a common dining area among friends and visitors and are provided with daily recreational opportunities including outings to sports and cultural events, parks, restaurants and shopping centers as well as on-site activities which include musical entertainment, games, horticulture, baking, residents' rhythm band, barbeques and more. Residents are cared for by staff who receive ongoing education in holistic care to meet physical, social, emotional and spiritual needs in an atmosphere of love and respect.
LONG-TERM DEMENTIA CARE: GUARDIAN GROVE COMMUNITY
Compassionate care for people affected with Alzheimer's disease and other forms of dementia is provided by staff specially prepared to manage challenging behaviors in a dignified manner. Residents of this unit are able to move freely, safeguarded by unobtrusive security devices. They enjoy an indoor wandering corridor as well as an enclosed outdoor courtyard. Residents and families are always welcome to join in with specially designed social and recreational programs which include baking, music and interdenominational church services.
